He's real for the bad stuff. Sorry for your losses. I remember when my grandpa stopped recognizing me. It sucks, and I can't imagine how it felt for his wife and kids. The mind is so fragile it's frightening.

i can understand your pain. i've worked with several people with dementia. i watched them sink into the lost world. if there is a silver lining, it is that, they don't know that where they are,  is not the place, they last knew. 

one lady in the home where i worked, had her husband commit suicide and her son killed in a car accident. neither of which she could process in her mind. sad stuff but the results, some of us will experience at some point. you have to try and remember them, back when they were healthy.
