The single payer system could possibly be in question (need more info to know for sure). Depending on what limitations any given system puts on doctors and nurses can lead some of the better ones chasing a better income somewhere else (city, country), or perhaps convincing students to take a different path, for fear that taking on a lot of student loans when your income may be less in your control. If doctors and nurses are overworked, in any system, more mistakes and perhaps even more likely poorer patient interactions can easily result. Often with the poorer countries, but Germany is leading the pack there. Doctors are also leaving the UK, with income being part of the equation. Something else I hadn't thought of until now that might have an impact is the changing age demographic: do we have enough doctors/nurses coming in to replace the ones retiring.
